Journey Patient Care is a record-keeping app for the family caregiver. It is not clinical software, it does not connect to any system you use, and it asks nothing of your organization.

The problem it addresses

The caregiver becomes the record.

Around 63 million adults in the United States are family caregivers, roughly one in four, and about 60 percent perform medical or nursing tasks while only about 20 percent have had any training for it. That is from the AARP and National Alliance for Caregiving report, Caregiving in the U.S. 2025.

Among dementia caregivers specifically, the Alzheimer's Association reports that 70 percent find coordinating care stressful, 66 percent struggle to find resources, and 53 percent find navigating the health system difficult. Coordination, not the physical work, is what they name first.

You see the result of that at the next admission, when nobody can say which medication changed, when the last fall happened, or whether there is a POLST.

What the family walks away with

One place instead of a folder and a memory.

Medications
Real schedules including alternating days, monthly doses, and on and off cycles, with doses logged as given rather than as prescribed.
Alarms
Full screen over the lock screen and audible through silent mode, with an optional step required before dismissal so a dose is not swiped away half asleep.
Symptoms
Pain and symptoms marked on a body map, and vitals and blood sugar charted over time.
Daily logs
Activities of daily living, continence, and sleep including naps and night quality.
Visits
Appointments, ER visits and phone calls, each with a summary the family can hand to a primary care physician.
Call log
Who was called, why, how many times, and whether it is still open. Useful for prior authorizations and coverage disputes.
Directives
Power of attorney, living will, DNR and POLST held as records rather than as paper in a drawer.
Emergency pack
One tap assembles allergies, conditions, devices, code status, current medications with doses and times, insurance, directives and contacts onto a single screen.
Family view
A separate passcode lets a sibling or a second caregiver see a limited version without seeing everything.

Why it is safe to recommend

It asks nothing of your organization.

There is no account, no sign-in and no server. The records live encrypted on the family's own phone and nothing is uploaded, because there is nowhere for it to go.

That means no data sharing agreement, no business associate agreement, and no vendor review. Recommending it creates no data flow to us or to anyone else.

We can never read a family's records, we cannot recover them, and we cannot hand them to anyone who asks. That is how the app is built rather than a policy we are promising to follow.

Please be clear with families

What it is not.

Journey Patient Care is a personal record-keeping tool. It is not a medical device. It does not diagnose, treat, or monitor anything, it gives no medical advice, and it does not replace clinical care or professional judgment.

It does not connect to an EHR, it does not transmit to a provider, and it is not a HIPAA-covered service, because it is the family's own record on the family's own phone.

It runs on Android today, and an iPhone version is coming soon. If a family is on iPhone, have them email us and we will let them know the moment it is ready.

Keep cards
We will send printed cards for your desk or your discharge packet. A caregiver can scan one and read the rest themselves.
Mention it
If you facilitate a support group, we will send a one-page handout you can put on the table. No presentation required.
Tell us what is missing
You know what families come back needing. If something obvious is not in the app, we would rather hear it from you than guess.
